Support Coordination

We have a team of support coordinators who cover both level 2 and level 3 (specialist) support coordination referrals.  As an NDIS participant, you may be funded for a Support Coordinator to help you:

Social Work

Social Work comes under Capacity Building / Improved Daily Living.  Our team of social worker can assist you to:

Social Work Assistants

Support To Thrive have social work assistants who work under the direction of the social workers to implement therapy plans to help you achieve your goals.

Positive Behaviour Support Therapy

(Comes under Improved Relationship funding in your plan) Support to Thrive have positive behaviour support practitioners who work closely with you and your family and/or service provider to assess behaviours of concern and/or harm. We then develop a behaviour support plan in consultation with you to try to ensure you can live the life you want and the people around you know how best to support you with this goal.

Employment Support

School Leavers Employment Supports (SLES)

Support To Thrive specialise in customised employment.  This is an approach where we explore the participants' strengths and interests thoroughly and work with the community to match them with the needs of an employer.   This may result in the creation of a microbusiness or employment with an existing company.

(finding and keeping a job)
This support is designed to provide assessment or counselling to assist you successfully engage in employment. Employment counselling may benefit you if you have experienced traumatic injury and need significant support (over and above a mainstream employment related service) to develop a new work pathway.

Employment Stories

Jacob’s Story

School Leaver Employment Support – Jacob is an extraordinary year 12 student who has special talents with small engines. Support To Thrive have been working with Jacob to create a customised employment plan. We are currently in the assessment and skill building phase. Jacob would like to find a way to use his skills and knowledge of small engines to eventually find paid employment or commence his own business. Support To Thrive have supported Jacob to advertise and interview support workers who have skills in motors and can support Jacob to build his own skills. Jacob then recruited his awesome support worker Al from ‘Looking for Ants’ who works with him weekly to service and upgrade small, motored items. Support To Thrive assisted Jacob connect with the commuity to seek donations of small motored items. Over the next 6-12 months, Jacob will continue to develop his skills and knowledge. Support To Thrive will continue the customised employment process with Jacob with regular meetings to see where his talents might lead to paid work.

Nyree’s Story

Designs by Nonii – These stunning items are the brainchild of Nyree. After having a stroke in her 30’s, Nyree was looking to return to the work force but needed to balance this need with the challenges she experienced post stroke. After working with the Support To Thrive Team, exploring her strengths, interests and dreams, Design’s by Nonii was developed. Nyree now creates a range of gorgeous personalised items for her customers across the Fraser Coast.

Mophie’s Market

Mophie’s Market is the brainchild of our youngest entrepreneur; Sophie. Sophie wanted to find a way to develop her skills with budgeting, fine motor and social communication whilst doing something she loves. She was also keen to earn money using her creative genius. Sophie works on making these gorgeous key rings in-between her busy school and dance program. If you would like to make an order, please contact [email protected]

Black Cat’s Craft

Tarlia is the creative director and owner of Black Cat Craft: Tarlia was looking for part time employment while she completed her studies to become a teachers’ aid. She was finding it challenging to find a role that met her needs, skills and strengths. She decided to recruit the Support To Thrive team under her ‘School Leaver Employment Supports, SLES’ funding to develop a customised employment plan. The Support To Thrive team spent time with Tarlia to identify her skills and interests and the types of work she might like to engage in. It soon became evident that Tarlia loved arts and craft and was very talented in this area. Her preference was to run her own business so she could work flexible hours, in her own home doing the things she loved. Soaps by Leigh

Leigh is the proud owner of ‘Soaps by Leigh’. Leigh makes homemade soaps in a variety of shapes, colours and fragrances. Support To Thrive assisted Leigh to identify her employment goals and connect with the right supports to help her achieve her dream. Leigh loves making soaps, serving her customers and creating new designs with her support team. Support To Thrive regularly monitor Leigh’s progress with her business and review the plan to ensure it continues to meet her needs.
